• Conference fees are in Australian dollars and include 10% GST.
• Full conference registration includes catering throughout the conference and the Reception on Thursday.
• Day registration includes catering on either Thursday or Friday; the Reception is NOT included.
• The Concession rate is only available to those who submit a photocopy of their Health Care Card or Student Enrolment with their registration form. (For those registering on-line, please fax this to us at (03) 9349 2230.)
To obtain the Early Bird rate payment must be received by Friday 22 May, 2009.
On-line registration/accommodation booking for those paying by Visa or Mastercard is now available from our
secure online registration and accommodation site.

CANCELATIONS
Cancelations must be advised in writing by 10 June 2009; there is a $110 cancellation fee per registration.
After that date, refunds will only be issued in exceptional circumstances, but substitute delegates will be accepted.
Please note that cancellation may result in loss of hotel payments.
VISAS
International visitors to Australia require a Visa or an Electronic Travel Authority. It is your responsibility to ensure
documentation is correct and complete before you commence your journey. Information on obtaining visas or ETA’s
can be found at: www.immi.gov.au
INSURANCE
Delegates are strongly encouraged to take out personal travel and medical insurance which includes loss or
damage of personal possessions and loss of accommodation payments and registration fees through cancellation.
